Eastern Shore Rural Health System, Inc. received the first-ever Health Care Transformation Award from the Virginia Health Care Foundation at its Cheers for Champions event on June 11 in Richmond. The award recognizes Rural Health for making changes that improve patient care by trying new approaches and strengthening what already works. The Virginia Health Care Foundation is a nonprofit that helps uninsured and underserved Virginians get access to care, medications, and health services by supporting clinics and community programs.

“Being chosen for this first-time award shows the hard work and dedication our team brings to improving care in our community,” said Rural Health CEO Jeannette Edwards.

That same week, Rural Health received two additional awards at the Aledade Summit, also in Richmond. Rural Health is part of an Aledade, Inc. Accountable Care Organization (ACO), where providers from various organizations work together to improve patient care and lower costs.

Rural Health received the Highest Transitions of Care Outreach Award for helping the most patients in Aledade’s Virginia market safely move from one care setting to another, like from the hospital to home, with clear instructions and follow-up.

Rural Health also received the Virginia market’s Diagnosis Resolution Improvement Award for the biggest improvement in identifying and addressing patient health issues so they no longer require active treatment or follow-up.
